We had the same experience as. M.V.The doctor said we can skip the flu shot but when the nurse said I had to submit some waiver forms, I quickly told the doctor to go for the flu shots. I wonder why the doctor is not telling his patients about the waiver forms?!

My husband & I went to him for INS medical exam. First he messed up the paper works with wrong names & signatures and then he asked us to just cross it and write over it. Are you kidding me? These are immigration papers. But he said it's fine.Second- he told us that it's ok not to take flu shots for these tests even though it was flu season. He said it doesn't matter. Two weeks after I get a call from his office that we both need to come again as you can't overwrite signatures. So we went there again. After waiting for an hour we got that paper and his office tells us that since you didn't take flu shots you will need to submit a waiver. We were even shocked to find out that there is quite heavy fee to submit this waiver and you should submit it only if you have strong medical or religious reasons, which were not the case with us. So we asked why didn't he inform about this before. But no answers. So we decided to take shot same day and get the new paper work. We had wait another 2-3 hours just to take shots and get the corrected paper work. Dear Sir- please respect others' time and don't give wrong information. It doesn't suit your profession.
