The developers did a fine job with this program. It moves the requester through the process smoothly and without delay. Kudos to the coders.
An electronic request for a simple form required a far to complicated web records request. Major time waster all because doctor owners will not pay a competent person to answer the phone.
Really easy tool for requesting medical records. You can skip the consent form and turning that on. Which is nice because that whole process takes awhile before they can even start collecting your health information. The only thing I wish this tool had was an option to print your preview report before you submit it. For your personal records.
They Application request form I submitted was easy and more hospitals should have the same Set up
St. Cloud hospital. This system is very user friendly. For older or non techy people it works quite well with a phone or tablet.🙂

Trying to photo your license was NOU an easy task. Just asking for the license would have been easier along with birthrate and last 4 digits of SS . Just my opinion 😉.
