Clear Creek Care Center's Online Review Report Card

I have been a volunteer at Clear Creek for many years. Shana and Maddison, activities staff and all staff members are always helpful and kind. Rusty keeps the building clean and beautiful. Their safety protocols are current. I highly recommend Clear Creek.4-4-21 My visit today with residents and staff was very good as usual. 😊6-22-21 I went to facilitate our Prayer Circle today. I'm thankful for my friends there, and the staff that takes very good care care of everyone!2-6-22 Always a great interaction with Activities Director, Shana, and my friends there!3-20-22 Shana is a blessing to the residents! She is so good at her job!

Pretty sad and extremely disappointed, when a nurse tell my mother she's too busy with 30 other people, in the facility, and doesn't have the time!Unfortunately, I had covid during two weeks she was there, so all the family need to quarantine. So I missed what was going on. But, here's what I saw in the last week she was there.My mom was unable to go to bathroom without supervision. When she called the nurses to help her on many occasions was told this..! especially the night crew. She had two showers in the three weeks she was there. My mother declined into a depressed state and I had to immediately bring her to my home to care for her. She laid in bed so long she had a bedsore on her back, and got a unitary tract Infection, I feel from not moving around..Immediately after my mom got out of CCC, her mood changed tremendously. I'd think twice if your thinking about putting your loved one there. I asked the day before bringing her home if in the morning they could shower her so I could have meeting my appointment with home care on how to shower her, when I returned the next day she was sitting in a wheelchair with her hair all matted, and she smelled horrible. They then wanted to shower her, knowing I would be wheeling her out in the cold.. it was very painful to see my mother in that condition.

PT & care (CNA PA DR RN) in general was top notch. Food, another story. They clearly tried but were out of basic stuff. Made a lot of physical progress in a short time. Came home w scabies that could’ve only come from Clear Creek.

Everyone is pretty good here the food is good too. A place where you can find new friends .

My mom was here for about three months, during that time she went downhill fast. They never did activities of daily living with her or any type of therapy that I seen. They could barely shower her once weekly due to lazy staff or being short staffed or both. After they put her on Compassion Care it seems they left her in her bed in a corner to wither away! Nurses have bad attitudes towards patients and family members! This place is sad and needs shut down, or needs better staff running the place. Also they take forever to answer patient call lights, someone could pass away before they get in there.

My mother has been there for rehab. The staff is so kind and caring. Do not let the siteof the facility fool you. It isn't updated, there is a faint odor. Put those things aside and allow the wonderful people there love o. Your family.

Clear Creek Care Center literally saved my life. I was at a point where my health conditions were making it hard to live independently. While in rehab I met my angle, my Case Manager Amanda, she kindly assisted myself and my family in transitioning to assisted living. She was very understanding that this was a difficult transition for myself and my family. The entire process was handled with love and care and set up for me to discharge to my new home when rehab was complete.While in rehab I met many wonderful and caring staff members that I consider friends. The entire stay was as wonderful as a rehab stay can be. I was treated with kindness and respect by everyone. The dining experience was wonderful. I highly recommend Clear Creek Care Center.

Kenneth MI give Clear Creek Center four stars.  I am very happy and pleased with the clinical and  supportive staff here at Clear  Creek.  The service and care has been good and I have no complaints.

I was there for a month of rehab a few years ago. Everything was great except for one nurse who was one of those people who shouldn't be working with people. She hated her job, the facility, the other nurses and CNA's and life in general. But everything else was great-one of the CNA's even gave me her phone number.

Going home is going to be the highlight of my life, seriously. I have had money stolen (and management knew it)and there was nothing done about it. I have reported this facilities HIPPA violations, their medicine dosing errors and this Corporations (Vivage) theft of patient personal incoming items ordered. Do not expect the State of Colorado kLTC investigations to save the day, because they are not. Not even your own Medical Insurance Carrier will give a rip what these people do to your loved one. Beware of Optum if your loved one is non-cognitive.
