Very clean and friendly place. They make you feel valued. I loved how he gave me options and choices on ways to help me. I have bad anxiety when it comes to certain things going to Dr is one of them. I'm glad to say I have found my primary care dr. I can say the only thing I don't like is I have to fast tomorrow for blood work. I go tomorrow and will write a review. I love how I go back to my dr office to get my blood work done

My former primary care physician is no longer taking my insurance plan, so I had to find another doctor. This was my first visit to Family Care Partners and I was actually surprisingly pleased with how I was treated. My intake assistance was so sweet and funny. I enjoyed spending time with her. Another staff member made certain I reached all the depts where I needed to be seen. The phlebotomist was so patient and gave me some very good advice about having blood drawn in the future, which I do appreciated. The office was large but very organized. Dr. Warren showed thoughtful concern and listened attentively, which made me feel secure. I was made to feel that my thoughts mattered. As I was leaving, I was convinced I had chosen the right medical practice.

Front desk is Awesome! Kind, quick, and helpful. Scheduled doc had to leave, so she got me in with another, no problems. Doc was great and able to pick up meds within an hour of appt. Super!!

They discriminated against my medical condition and therefore deny me service... then had the audacity to send me a letter stating that I moss my appointment and that I should give them a call to reschedule yeah right ... im not going to HINDER my health to make other THINK and FEEL safe because my well being, and safety is just as important as the next person... due to medical discriminate i would give it. Very friendly clean office. Wait time isn't bad. The PA Jennifer Anderson was patient kind and had respect during the exam. Listened to my needs sincerely. Even helped me get testing done sooner. Def recommend.

I had an appointment today with Jennifer Anderson at the Fleming Island office. I have to say she is wonderful. The entire staff is wonderful and it is so nice and clean in this office. The wait time was very short. I've been coming here for several years now... probably 6 years and I love this practice. If you need a physician, I highly recommend Family Care Partners in Fleming Island for your care. You will not be disappointed. Thank you, Robin Pelliccio

Dr.Afshar has been my Dr for a while now and he's really amazing. I go in and when I leave feel better than when I see my therapist.. it's not all about medicine, he's big on self love and mental health. The world needs more doctors like this .. .they let you talk , and help meet all needs you went there concerned of ! Shout out to him!!!! -hailey radford
