Avante At Lake Worth Rehabilitation And Skilled Nursing's Online Review Report Card

I have a sister who was recently admitted into this facility, "Avante at Lake Worth" 3 days ago. She is a "Trek" patient, and have special needs and is someone who needs to be watched often. The nurse "Silvana" who I met that night she was admitted assured me that my sister's medications needed to be inputted into the system. She lied in my face about inputting them into the system. They are not trustworthy. My sister had to be rushed to the hospital the very next day due to not having any of her medications. The entire time my sister was admitted there with NO medications giving to her. Very sad and the whole staffing team that night communicated to me that she was in good hands, and there was no need to worry. Wow! My sister seized all through the night going into the next day from the facility not having any of her medications. Poor management. I'm not a person to exaggerate about things, but this first time experience my mom and I had was "Terrible".

My mother has gone here TWICE for respite visits and my biggest problem with Avante is no one answers the phone up front. I call and call and it takes a good 3 hours for someone to answer the phone. Another huge issue is the front receptionist does not take messages to my mother nor to her nurses. And much worse than them not answering the front phones, try speaking to a nurse!! That rings a few times and hangs up on you!! How can a facility get away with not speaking to a family member after they have called repeatedly???? I have yet to ever speak to anyone that cares for my mom while she’s there, only the front desk when she decides to answer it. There is more excuses than answers… And the only “excuse” I get is they are short-staffed, interesting enough if you go to their job website, they are not hiring!! So the 2.8 ⭐️ are quite accurate-

This is the worst place I have ever seen. My husband was taken to this rehabilitation center and they almost killed my husband. The nursing staff never contacted me about my husband’s care. They took him off his medication in April and never put him back on it. My husband’s blood is out of control since taking him from this facility. He was abused my the staff. He was denied food. They didn’t do any physical therapy with him. He just laud there and suffered. my husband had neck and back surgery and those people were supposed to do physical therapy and occupational therapy and all they did was take the money from the insurance. He spent eight months in this facility and they almost killed him. What kind of doctor tell a patient when your blood sugar get to 300 then we can give you medication. I will not stop until this facility is closed down. I will find someone who will listen to me about this facility. The staff is incompetent.

The worst place I have ever been too my dad became soo depressed here they basically didn’t act concerned about his well being he was only there to get rehab basically sent him to the hospital for no reason and on top of all that stole his protein drinks and had the nerve to ask me for a receipt I don’t have to prove a thing of what I bought for my father to get better I’m so glad he is away from this awful place..would not recommend to anyone I know.

I want to take a minute to recognize an employee who goes above and beyond at Avante. Ms Lola at the front desk is ALWAYS ready to take a few extra minutes when I am trying to advocate for my father from out of state. After months of her helping with tasks from insurance documents, to contacting the various departments; today she went even farther to take his cell phone to him after he was admitted to the hospital. I so appreciate knowing that she is so compassionate to the residents!

My mother in law was a patient at Avante at Lake Worth and was recently discharged home. She was not able to care for herself due to a massive stroke and Avante rehab and nursing staff helped her become stronger and back to herself again. The dedication and commitment of this team has been so amazing. She even enjoyed the food, which being a healthcare worker, I know is always a major complaint in facilities. I highly recommend this facility if you would like your loved one to get the best rehab and nursing care. They truly care about their patients :)

The only staff worth anything are the ladies at the front. Nurses are horrible and they are the ones taking care of your loved ones. The office staff are pretty useless too and not helpful. Trying to get someone to answers questions and provide information is a joke. Do NOT send your loved one here.

I’ve been in and out of this facility many times. One memorable moment was when a “code” was signaled (a patient was in immediate need of attention to prevent serious decline that may call for 911) & all staff close by were merely walking without any sense of urgency. Many looked confused & unsure of procedure. Even if a patient was a Do Not Resuscitate, the staff should’ve responded way better.

If you have done so much helping patients be safe from the Covid 19, tell me why my sister Janet Polak was there a short time and now has died from it. Don't think it is such a save place.So sorry the Hospital picked such a horrible place for her to go to.
