Great and caring Doctor and staff.

Dr Crouch is a great caring Dr. This is a for real affordable health care. Very low cost for blood test etc. Cheaper than our Dog Vet. He has 4+plus Stars, lets get him to 5 Stars. Also, just one more thing, Dr Crouch listens to us & shows great concern to get us well. No waiting more than 5 minutes when you arrive, Also a good thing, he doesn't over book. Please help get the word out for a great Doc. God Bless us all. Tom

I am no longer a patient of Dr Crouch. It is unfortunate that he no longer accepts Medicare patients. We have started seeing his nurse Candice and find that she is very thorough. Quite pleased with her total performance.

Becoming a Member of Epiphany Health has been a great decision. Having a Dr and staff that cares so much about our family is a blessing. Dr. Crouch spends time with us as patients, he listens, cares and explains everything. The staff is superb.

The doctor is great however... I had one appointment with the doctor where he prescribed new Asthma meds, when I went back for a follow up and refill on my prescription they told me on the day of my appointment that they no longer take self pay patients and that you have to sign up for their plan where you pay them $65 a month for a year, whether you need to see the doctor or not. Would have been nice to know that before getting started on a new medication and then being told I couldn't see the doctor with out being strong armed into signing a contract. $65 a month or $780 a year. The office staff shook their head and acted as if I was crazy for not wanting to pay in advance for visits I wouldn't need. Why didn't anyone say this was their rules before seeing me and starting me on new meds n the first place?

The doctor is great but if you don't have insurance and would like to be self pay then you can forget it. I had one appointment with the doctor where he prescribed new meds, when I went back for a follow up and refill on my prescription they told me on the day of my appointment that they no longer take self pay patients and that you have to sign up for their plan where you pay them $65 a month whether you need to see the doctor or not. Would have been nice to know that before getting started
