I too have had my fathers primary fax the referral 3 times. This time the primary doctor called this one’s office to see what was going wrong. That was three days ago. Still nothing. I would love to find Dr Hazday.

I tried to schedule an appt, my doctor faxed the referral multiple times. They said they would only accept a faxed referral from my doctor. They said they never received anything and would not follow up.

On December 9, 2009, while in HRMC, for over night observation, I suffered a massive heart attack. I was in shock and complete denial when Dr. Jara, who was visiting a recovering patient in the room next door, came in answer to the code issued by quick thinking nurses. A stent was placed in the main artery after a large blood clot was discovered. I am coming up on 10 years as a survivor of a major heart attack. Thanks to my wonderful and caring doctor I am still this side of the dirt. Having several medical issues during my 75 years on this earth, it is reassuring to meet a doctor such as Dr. Jara. Dr. Jara saved my life and he would say it was not just him but his team. It was Dr. Jara’s skill and swift action that truly gave me the gift of life. For that I am forever grateful. Becky M. I have seen Dr Jara twice in the past six months. I have found him to be kind, caring and engaged. He actually listens to what you have to say and answers questions in a clear and concise fashion. If I ever have a heart attack I want him to be called!! I referred my entire family to him. Sara,his nurse is also awesome. She's the one that really knows what's going on with everything. Very efficient. And everyone there is sweet as can be. Excellent experience all around!!
