I was suffering from frequent recurring sinus infections. Dr. Moak and staff quickly got a me on the schedule for surgery to remove a polyp. Everything went well. I had another sinus infection after that. They had me come in to evaluate the problem. Took a biopsy to find a severe bacterial colonization. A specially formulated prescription nasal rinse was was ordered. That did the trick.

I was referred to dr Moak in 2021 after having trouble breathing out of my right nostril. Dr Moak preformed surgery to repair this condition however afterwards I had blockage breathing out of my left nostril. In 2022 he preformed a second surgery to correct it however after six months and several visits breathing out of my left nostril became worse. At that time he said there is nothing more I can do for you and stopped seeing me. Today my situation is worse than before I ever visited Dr. Moak only now the difficulty breathing is in my left nostril. At this time I have no confidence in his ability as a surgeon for this procedure and feel as if he quit on me.

Went in for lesion of tongue. Doctor explained possibilities as well as possibilities of treatments based on biopsy results, also was quick to get biopsy scheduled. Professional and kind manner.

Doctor Moak was very diligent and professional. I had an object removed from a previous rhinoplasty done by another surgeon. Thank you Doctor, keep up the great work!!

Dr. M, for a younger gentleman, has old fashion values when it comes to treating patients. He looks you in the eye, shakes your hand and listens to you. He has impeccable credentials and his recommendations are ethical, which may not reap the best monetary award from insurance companies:) I am 75 years old and a practicing psychotherapist from New York City. Having lived this long I had some really great doctors taking care of me throughout my life. I trust this doctor and I’m reassured that I will always reap the benefits of my association with him. In addition, his staff is friendly and down to earth. I applaud lake ENT.

Absolutely the best!!!! This photo was taken today a few hours after my stitches were removed!! It’s shiny because I have antibiotic cream on it, but the cut is barely visible already!!!! I am extremely pleased I found Dr. Moak!!! I unfortunately had a small cyst ( I picked and picked a blemish), after a terrible, terrible experience and butcher job, w Advanced Dermatology, I walked out and looked across the street!!!! I can’t say enough about the wonderful staff and the Dr.!!!! I can’t wait to see my face in a few weeks!!!! Ty!!!! Ty!!! Ty!!!!!

I went there with an earache & stabbing pains I had for over 2 months. He immediately said it was a TMJ issue. I don't have TMJ (verified by my 2nd opinion ENT). He would not look any further. I get the feeling he tells everyone it's TMJ immediately & they accept it. I'm glad I didn't because it looks like it is a nerve issue

I made an appointment at Lake ENT/FPS for a dull earache with sharp stabbing pain I had for 2 months. I was scheduled with Dr. J. Samuel Moak, III. He told me immediately I was suffering from TMJ. I knew I didn't have TMJ. He wouldn't check me any further and kept insisting on TMJ. He touched my jaw, said "ok maybe you don't have TMJ - see an ear surgeon". He gave me no recommendation, told me to check with my insurance co. Later I find out Moak is a plastic surgeon. Why was I scheduled with him?? I decided to get a 2nd opinion. BTW: After checking my jaw, he immediately reassured me I did not have TMJ.
