Chris Crooker's Online Review Report Card

On 61521 I faxed they would not provide an email address an Annual Physical Confirmation Form that needed to signed. This form is required by my wifes employer to avoid a $1000 premium surcharge.I called on 62121 to request a status and we discovered that the original form did not have my name on it the fax cover sheet did?. I refaxed the form later the same day.I called again on 62421 and was told the form was sitting on someones desk waiting to be signed but it should be done that same day.I called again on 62821 and was told the same thing. When I attempted to escalate the issue I was told by the lady in medical records this was not her job. I waited a few hours and called back and was told the same thing by the receptionist. I explained the importance of getting the form submitted by 63021 and that did not seem to matter at all. I asked to speak to the manager and I was told that I was being argumentative and the receptionist hung up.I have been a patient for almost 5 years and this is the first time I have ever requested anything and this is the service I get

I’ve been helping my mother with her health and I have had to send no less than 17 messages just to get a diabetic monitor called in and it wasn’t even the right one Then they didn’t call in any of her insulin which I asked for in the same message because her insurance changed and they didn’t cover her old script. When she had Covid neither he nor his staff returned any calls or messages until DAYS later. Even afterwards the complications from having Covid they couldn’t get her in to verify an issue. I had to argue for an antibiotic and steroid because they wanted to see her but had no appointments until almost a month later....how would that help by then? My family have been patients with Dr. Crooker for over twenty years and I am fed up with this office if I could I would give them zero stars. I will be finding a new PCP.

I must admit that I am a handful. I was accustomed to being seen by a nurse practitioner. Once that person retired I was placed under the careof Dr. Crooker. Trust me I was a bit apprehensive. Dr. Crooker has gone above and beyond every single time I have seen him in person communicated with him via patient portal or have spoken to him over the phone.My life is very trying and I could have not have asked for someone more caring and attentive than Dr. Crooker and his staff.They deserve an awardJust think of what it must be like for them to handle everything that comes their way on a daily basis. God bless them

Dr Crooker is great just cant get a appointment with him. Have to book way ahead. Lab work forget it cant get anyone to call you with results.

There is a lack of care with this practice. Prescriptions were given to me that could adversely affect my medical conditions. Antibiotics were prescribed that were contraindicated for my medical history. When I messaged the doctor about the medicine he told me hopefully you can get better on your own. The office does not call with test results and recommendations.

Dr Crooker and his entire faculty are the worst. Your personal business is breached. He shouldnt even be a Dr.

The only good person at this office was Richard. Since he is no longer there it sucks. Dr Crooker and his entire faculty is awful. Uncaring and sketchy stuff going on in that office. Leaving to go somewhere else after 17 years.

Great doctor always very helpful and take the time to explain any questions I feel very lucky to have him as my doctor. Thank you Dr. Crooker
