I had significant difficulty in reaching out to the office for obtaining my physical exam form. The sole reason I went to the office was to get my physical form filled and I paid them 300$ just to fill the form. I asked many of my colleagues they all had difficulty in reaching the office by phone or fax.

I was referred to Dr. Schultzes for my son Who aged out of his pediatricians office. He has severe mental retardation but otherwise is perfectly healthy. He refused to take him says he needs a specialist. Completely ridiculous and makes life much harder for people that have it very difficult anyway.

I WENT THERE FOR MY TEST APPOINTMENT AND THE DOCTOR DID NOT COMPLETE THE FORM AS NEEDED DESPITE ME PAYING THE FEE. THE IMMIGRATION HAD ME RE DO THE TEST AND THE OFFICE HAD ME PAY A FEE THE SECOND TIME. THIS FALLS UNDER UNPROFESIONALISM.
