They wanted to take me off of most of my meds all at once and these meds are crucial for me to have they all have made a positive impact on my life and I cant go without them. None of them are narcotics either. They claimed I was missing one pill when I actually had a bunch extra cause I didnt take it everyday. Plus going every week is a bit much when they wont even fill my meds.

Ive been going to Clean slate for almost 3 years and have been completley sober since then I am so HAPPY that i found this clinic The staff has changed which is the only bummer..miss the old staff but hopefully the new staff is as pleasing as the old Loved Cynthia and the whole staff crew Thank you so much Clean slate for being there for me when I was facing my demons and helping me through the way to be clean not just for my family but for myself 🙏💗Proud AndSoberBratAlcantar

For addiction to opiates recovery using suboxone treatment Clean Slate is A+++. Very caring knowledgeable friendly helping staff. The doctors are highly trained and specialize in opiate addiction. Changed my life. Let them change yours.

Best medication assisted treatment in fort wayne.. i have been to several outpatient clinics and cleanslate’s staff is by far the best i have encountered.. Angel is always very welcoming and encouraging. Be sure to speak with her if you join the program

My name is Anna Greenhill. I have been in treatment for a while now and this is by far the bestkindest treatment center I have been to. The staff are very kind and do not judge you the doctors are also very kind and always full of information to help you reach a better place. I would definitely recommend this place to any addict that wants to get clean and is tired of struggling. Thank you to all of the staff at clean slate for making me feel so welcomed into “the family”

Clean Slate is a God sent outpatient program that proves recovery is 110% possible Whether it took near death experiences hopelessness losing the ones you love a life with what seemed like never ending relapses and the millions of struggles addiction lays on us no matter what your story was while in active addiction Clean Slate can give every opiate addict hope for a new beginning that starts with sobriety. You are not defined by the person you were in addiction whether it consumed a few years or half your life. Relapse is not failure you just have to jump back up and keep trying. The staff at Clean Slate welcomed me with open arms not once giving me any reason to feel judged or too far gone to change. The amount of care and support I receive from these earth angels gives me a reason to enjoy coming to every single one of my appts. and I’m so proud to say that I chose sobriety over addiction with the helping hand of Clean Slate and I hope you do too
