Ive been advised to not put anything at all but Im the one paying the fees. So here goes. Clean Slate knows whats up. Long story short I called to confirm the whole point of my appointment two weeks prior by contact via phone and in person prior to the appointment. I was lied to on a recorded line and y character was def......drug thru the mud and called a liar.While being told they would not help me I was being prescribed meds for kickback during all this with no consent all so this nurse could still try and boll me. Before i left we myself and clean slate agreed I would not be billed for this visit because of my contacting several times prior. Now months later after my neg. review they break the Consumer Review Fairness Act and start texting me up to 8 times a day asking me to come back and when I said dont call me again they send a text saying my itemized bill is ready online. You tell whats FAIRNESS about that.
As I read these reviews I am shocked to see such negative comments left that I would 100% disagree with This place has kept me off of opiates for almost 6 years....the staff has the upmost respect for thier patients and dont throw u out when u make mistakes they help u thru it instead. They are encouraging and supportive of thier patients doing all they can to help....they make u feel at home and like they care instead of acting like theyre just there for the paycheck. I would totally recommend this place to those struggling with addiction I know it saved my life and it can yours too
I see jasmine is writing in reviews for this place and she is a emoloyee here. If u listened and treated people w more respect u wouldnt need to write a review urself. Hope i get an apology next time i see u for calling me a liar and making me go even more days w out meds and risking my sobriety now that i had to have my blu anthem transportation call and tell u exactly what i told u happened and i had to miss my appt from no fault of my own. This place is suppose to welcome people who have had a rough time w life and b understanding to their patients and atleast listen to what they need to say before making their own assumptions.
If u dont have to go to this location dont. Most of the staff think they are above u and dont need to listen to u. I was going to another cleanslate for over a yr and loved all the staff no matter how busy their day was they still made time for u. Unfortunately i had to start going to muncie and i have had bad experiences from different staff from day 1. Some of them should b reaching out for help themselves.

Cleanslate helped get my life back they treat you like a person and have some of the most understanding and caring staff I ever met
Its a goof place. They care about you as an individual they help guide you back on the right path and hold you accountable.... Which is what I need.
I think a lot of people who are going through a rough time with drug abuse can get a lot of help out of this program because of the way they treat you with respect and dignity in a very friendly way.
Well I have been calling and calling and never get a answer or a call back I dont understand this is a doctors office and cant never get ahold of any one
I have never met my prescriber and they have charged 145 dollars a week in urine screens. Its s scam. Dr. Gannon doesnt know first rule in medicine do know harm
