This doctor has severe mental health issues. While initially kind on the phone when I saw her in person she was a completely different personality. She was domineering, verbally abusive and forced me to sign release forms that were not completely filled out. She said she would do it later and I could not have treatment unless I signed, even though I explained I had already filled them out but ...
