Dr. Cook evaluated my 7 to granddaughter and was spot on. She responded well with him and has been very difficult. I know he will be able to help her turn around. I will drive from Lebanon to Ft Wayne to have her seen by him. He's that good!

I have been a patient at Arden Family Health for almost 20 years. Today’s visit with Dr. Jason Cook was very unprofessional, rude, and hurtful. The rest of the staff was courteous and caring.

I have experienced about 90% relief from chronic migraine! Dr cook is straightforward and kind. He took the time to thoroughly educate me about migraines, and get to the bottom of what were causing mine in order to find the right treatment plan. I have also had a very good experience with technicians, office staff and have received stellar phone support. His office has walk in hours for acute headache patients, which is incredibly helpful. I have never received such high quality medical care.

I went to Dr. Cook with severe gut trouble. I was swollen badly. He told me to take GasX. After almost a year of suffering, I ended up with a colonostomy. I was not impressed with his diagnosis.
