Been waiting for new prescription for a month! No response

Been waiting over a month now for him to respond to my pharmacist or myself about my prescription. Insurance would not cover the brand he prescribed and just need him to prescribe a different brand. He has had no contact.

To whom it May concern , Please have a managing partner or your CFO contact me by mail with a direct contact number . I had a short consultation to for evaluation for Surgery . Then scheduled appointment for further treatment and cancelled the appointment weeks before it was supposed to happen . I feel my insurance was fraudulently billed for treatment that never happened and I am getting non stop phone calls from your office for a deductible for a surgery that never happened.. I reviewed my insurance statement as if some treatment was actually done and was billed for it . I just got off the phone with a collection rep from your office to whom I conveyed my concerns to have them dismissed with a response of that's not how your insurance company sees it and you are going to collections. I was called before the surgery by the same dept to collect a deductable in advance prior to the surgery (this immediately prompted me to cancel any further Business) This is poor business practice and I will post my experience on any feed back sites I can . And the DOH if necessary.

Dr. Lundak is so Wonderful! Wish I would of known about him 3 mths ago when this all started! I have been in such pain for about 3 mths. He explains everything in such great detail so that you understand what's going to happen or what has already happened! After surgery he explained everything to my husband in detail. So very pleased with Dr Lundak. He gets a 20 in my book!

I went to Dr. Lundak when the pain was about 6-8 after doing simple lifting and even thoughts of sex caused pains. He had me try medicine for a while and that helped somewhat to reduce the swelling and pain. It ended up needed surgery and he was good through the whole thing.
