David Nieves Md's Online Review Report Card

I AM A PHYSICIAN with 23 years of clinical experience, and I will defend my medical colleagues to the death! This is the first review of a physician that I have ever written. I was compelled to write it. I was very disappointed with the medical care that I received from Dr. Nieves. I would like the Doctor to hear constructive feedback regarding his care. I am writing this review respectfully and thoughtfully, and not spitefully. I have also previously written a careful letter to the Doctor and the office manager. The office manager and medical assistants were polite and respectful. The manager called me after receiving my letter and was very apologetic and professional. All the folks that work in this office are excellent. The office is extremely well run and clean. This review focuses solely on Dr. Nieves. I saw the doctor in regards to psoriasis and acne. The medical care was severely deficient: 1. The careful medication review by the MA was totally ignored by the Doctor. It was clear to me that he did not review the medication list. An association between one of my medications and acne was ignored. Historical data about Acutane as a treatment for acne was never obtained. Possible drug interactions were ignored. 2. A well-described side effect of one of my medications as a cause of psoriasis was ignored…..The Doctor simply stated that “no one knows why people get psoriasis”. 3. Doxycycline was flippantly prescribed with ZERO discussion about possible side effects such as sun sensitivity, and no review of my medications for possible drug interactions. Doxycycline has a significant drug interaction with one of my medications. 4. A discussion of possible biological treatment for psoriasis took 5 seconds, literally. 5. A topical steroid for scalp treatment of psoriasis was prescribed, but there was no mention that when this medication is used in patients with a particular underlying medical condition, a dangerous adverse clinical outcome can sometimes occur....……Several months later, this adverse clinical syndrome did in fact occur. THIS CLINICAL SYNDROME CAN SOMETIMES BE LETHAL. 6. Steroid injections for my scalp were performed. The doctor spent under 60 seconds doing these injections with zero discussion of RBC's (risks, benefits, complications) related to my underlying medical conditions. These injections can very rarely produce the same adverse clinical syndrome as stated in #5. 7. The total duration of the office visit was approximately 10 minutes…..Given the complexities in regards to discussion of psoriasis, acne, performing injections, a medication list, prescription of two medications, and a discussion of biological treatments for psoriasis, 10 minutes is grossly inadequate. The infamous stereotype about dermatologists is that they are interested only in generating revenue via cosmetic procedures, and that they are not "real" doctors, meaning that they don't practice "real" medicine. I have previously defended my dermatology colleagues to the death in against this stereotype. No longer! In regards to Dr. Nieves unfortunately, this stereotype is dead on. As a fellow physician, I am deeply disappointed that Dr. Nieves did not perform the most basic medical tasks that all physicians master during medical school: ALWAYS REVIEW THE MEDICAL HISTORY. ALWAYS REVIEW MEDICATIONS AS THEY RELATE TO THE CAUSES OF DISEASES. ALWAYS REVIEW POSSIBLE DRUG INTERACTIONS. Failure to do these three tasks at every single patient interaction can cause adverse patient outcomes and can occasionally cause deaths. In summary, the care provided to me was substandard. In fact, it was dangerous. I am saddened that a distinguished peer of mine, who has been so expertly trained, would provide such deficient care which wound up causing dangerous clinical outcomes. As a physician with 23 years of experience, it's been my lifelong goal that all people receive only the best medical care. In this case, the care was down-right dangerous. Doctor barely spent 5 minutes with me. Did only a very fast visual exam of my body. He had not read my chart and didn't know I had a cancerous spot removed some time ago. When I asked him if I should make an appointment for my annual next year he said no you're not a high risk patient and left the room. I thought everyone was supposed get an annual skin exam

Fantastic doctor

This doctor has terrible bed side manners. Ruthlessly announced I had rosacea by looking at my face for all but 2 seconds. When I tried asking more questions about it and expressed concern over maintaining an anti-aging skin care routine, he scoffed at me dismissing my concerns as vanity. Will never go to Windsor dermatology or this doctor in the future.

Staff was nice and very friendly the facility was beautiful. But the doctor spent all of three minutes with me. It seemed like he just rushed me out and I forgot to ask a question.

Appropriate Covid precautions. Almost nobody in the waiting room. Saw the doctor quickly. He seems very knowledgeable. At the end of the exam, he asked if I had any questions. I had one that he answered quickly. He then left the exam room. He was in and out in 5 minutes. I’ve had similar exams with other doctors that took much longer. The feeling is that longer exams are more thorough.

Very good experience. Handicap entrance and accommodations are great no problem getting in with a wheelchair. Dr. Nieves is easy to talk with and communicates well. A very competent and friendly staff.
