Pinnacle Multicare Nursing & Rehabilitation Center's Online Review Report Card

Was accepted for physical therapy and rehab....so as long as I approach myself in person,I do not want nor need anything negative with me under any circumstances

this has been a very hard thing to do like putting our parents in a good place. Pinnacle's staff on the 3rd fl. made my choice easy even when I pop in on them there on point. thank you

So my personal experience with this nursing home my uncle passed away due to their negligence! Lying that a Cather was change yet IT WAS NOT ! Not only that at one point he was sitting in the day room and they were sitting eating while my uncle was PASSING OUT DUE TO SEPTIC SHOT ! The CNAS always have an attitude and the 3rd floor manager needs to be spoken to about her attitude! But i will say i appreciate the CNA Kerry she was the ONLY one came and tried to help my uncle while another CNA was ready to let him choke saying he had a DNR band on! I would NEVER recommend this facility might be clean when you walk in but DO NOT TRUST your family in here ! As i wish I could bring my uncle back i pray this never be the case for anyone else in the future !

My daughter in law was at this rehab. And she lost her life on June 2nd. Because the pca didn't check on her and her oxygen tank ran out. This is very sad and my family is grieving. She was only 54. That pca should be accountable for her actions. The staff should be more considerate. My daughter in law always had to yell for service and when they come to her room they have a very bad attitude. The owner leaves his number for my daughter to address the issue. But he never answers or call her back

Kim D Of course I am grieving to lose my Best Friend at 54 years old a beautifulperson & soul, is heart breaking but I want to say this facility really need to send staff to professional training on how to treat patients and their families....this is horrible the service received the way staff speaks to you and why friend felt like staff was mean to her is unacceptable.......you have the responsibility of making patients feel safe 😢 that didn't happen in my friends case.....God see everything 😢

If i could give this rehab -0. I definitely would. My Wife been at this rehab for months. Keyword rehab. She had to yell for the staff to come help her. The staff is very lazy and have no compassion. On June 2nd my wide passed away because of neglect. She needed oxygen and they allowed her oxygen to run out. I came to visit my wife and found her gasping for air. And her oxygen tank on empty. My wife was just 54 years old and a fighter. They left her in the wheel chair and didn't even bother to check on her....God see all..... please don't bring your love ones to this facility..... one week later and the owner leaves a number that no one ever answers. Why leave the number. I would like to know who was the pca that allowed my wife life to be taken at 54. By not checking in on her and allowing her oxygen tank to run out.... she should be accountable for her actions

Staff was welcoming and caring helpful and informative .

My Brother stayed at this nursing facility and passed away shorty after arriving,during that time hes care was 50/50 with some nursing/ and techs caring and not caring example he needed to be feed and they would never feed him until we caught them and complained never had water in hes room never came to check on him as scheduled. Never took him for hosp vist as planned. Never moved hes body postions as planned we witness this while visiting had to call them on serveral occasions. But the most disturbing thing as when he passed they lost hes gold cross on hes neck and some other belongings, which we waited for over and hour and are half as they were searching to no reveal. I spoke to social service on many occasions with no call back and the director of the hosp who said they would look for hes belongings and give me a call that was over a week ago still no call. So I decided to let people know do not use this nursing home. Half of the workers care the rest could not give a care for your loved one. If i could of did a do over i would of pulled him out right after the first incident.
