Autumn View Health Care Facility's Online Review Report Card

My mother is currently a patient there in the outpatient rehab facility. The PT and OT have been wonderful. However the food is terrible and the facilities can be a little cleaner. The nursing staff isn’t that great either. They ignore patients needs and have terrible bedside manner. The administrative side is wonderful, they will call back and answer any questions family members have.

My grandmother was there and the care she received was excellent. The staff was very helpful and the facility was very clean. I would definitely recommend this place.

I have had several family members in this facility. Both long term and rehab. The care has always been great and the facility was clean and the food was good. I would recommend this facility to anyone who wants their loved one to be taken care of properly.

My grandmother was a patient at Autumn View and had great care there. The nurses on rehab were patient and understanding with her and our family. The facility was clean and the staff was very friendly. I would recommend this facility to anyone looking to put a loved one in a health care facility.

My husband had great therapists at Autumnview.But the nursing care was deporable.Waiting over 2hrs for CNA's to return after answering call light.To say they needed more help to assist him.We had to call Nurses Station many times for better care.My husband was there for 17 days.He lost major weight, bc food was not appetising to him or many othersHe also was abused by a CNA Verbally & Physically.Im glad he spoke up.Bc he may have ended up being advocate for others who may of been afraid of Blessed.It was investigated into.Aide was fired.Main problem is many of these workers that work at Nursing Homes, are totally underpaid . So private owned nursing homes dont pay enough, help isnt trained enough.With Covid in the past. Many have lost great employees.But Blessed was not one of them & for thatSo you get pick of the litter.My husband could not wait to leave Autumnview RehabBut he did appreciate Smiles, Ryan & Christie..Therapists

You did not accommodate my grandfather's disability and gave him bedsores. Worst "rehab" ever. Stay away.

Went there after breaking both ankles .Some of the staff were too notch.But while I was sent out due to medical issues,I was robbed of money, gift card and personal belongings.

Somehow my review was deleted after giving entire details. Staff and therapy staff were wonderful VERY POOR housekeeping I found it dirty and unacceptable. In this time of Covid, housekeeping should be hyper vigilant. It’s almost non existent. Food was terrible.I would never return, nor would I recommend it. I only picked it because I was under the false impression that it was a 5 star facility.

If you don't want your family member to get COVID-19 don't use this facility. A report was made to this location about a CNA that traveled to Florida and failed to initiate the New York mandated quarantine putting all the staff and patients at risk at this facility. This facility stated it's not required ...meanwhile this employee can be infecting everyone. This LONG term employee could care less about your family's health.

The staff is great I can't say enough about how caring and concern the staff has been in my recovery. I would like too say a special thank you to Paige, Melissa, Rachael(float) Kelly,and Mike on nites.Your therapist are great I was not the most cooperative patient in my rehab but with the persistent of Kara PT her encouragement and positive feedback truly help. My Ot therapist Emily has a little motivation in her approach in getting the job down . I will miss both I thank you so very much.Now the department that maintains your equipment, beds needs improvement. I had a broken part of my bed and sight to get it repaired meanwhile I had to increase my water pill to reduce the swelling in my FEET. Could not wear my sneakers so I thank you.
