Called Landmark on 5/18 at 7:30AM explained the issue I was having five minutes later got a call back from a nurse who went over thoroughly what the issue was. At 9:15 Nurse Practitioner Amy was at my front door. She addressed my situation sent in a script and forwarded all the details to my specialist. I can’t say enough good about Amy and Landmark. Amy was very knowledgeable and personable! Home visit was kind, courteous and professional with issues covered thoroughly. We had ample time to have questions answered and concerns addressed. These home visits provide for a relaxed, non-threatening environment. Will continue using this service for my family member.

Cara Wickham, my case manager from Landmark Health who does a home visit with me every 3 months, came yesterday - January 4, 2022 at 10:30 am! This was her fourth visit to my home since last March 2021! Cara is very knowledgeable, professional and an excellent listener! She brings my health records up-to-date and takes my vital signs. She was very pleased with my progress on health issue improvements over the past 10 months, particularly my weight loss and my A1c level is now at 6.0! I will see her again at the beginning of May, but she is available for any questions or consultations in the meantime. I am so pleased and grateful to be receiving these excellent home services to address my health issues and concerns, not only from Cara but also her several colleagues who have made home visits when I needed assistance!
