If I could give this place a 0 I would, it’s HORRIBLE! My father was discharged there for rehab back at the beginning of Jan. He was placed in the Covid unit for 3 days then moved to the general population. Once moved to his room, it took days for rehab to come in and evaluate him. Once they evaluated him, they never returned to do rehab like we was told. He was never turned and developed several bed sores on his behind. They came in his room during the night and placed a catheter for no reason. He was sent to the hospital (where he received excellent care), was discharged back to this nasty facility after a week of being in hospital. When he arrive back at facility I questioned the nurse on turning him and why he had a catheter. The nurse tried to say he developed the bed sore at the hospital, uhhh no lady he did not, he had them prior and she also tried to say the hospital placed the catheter, again no they did not. I told her no this was all there and done prior to his hospital visit, she then couldn’t give me any answers and left the room. While there, the room was never cleaned as I watched a small piece of plastic that laid in same spot on the floor the whole time he was there, no soap in his bathroom and the toilet was nasty. While he was there, he did not get the care he needed, only continues to decline. Our family doctor seen him and told us to get him out of there because if not he would not make it out alive. On the night before we picked him up, they “forgot” his dinner and when they realized it, the kitchen was closed. So instead of seeing if they could make a sandwich or something someone went and got him a bag of sour cream and onion chips and cheese it’s for his dinner at 7pm, he has some heart issue so this probably wasn’t the best choice for him. We decided to have nursing and rehab come to the house and he’s doing so much better than in that place!!! DO NOT SEND YOUR LOVED ONES HERE!!

I worked at this facility as an state tested nurse aide from 2011-2018 . The managing employee/administrator at the the did an phenomenal job managing the company as a whole. He encouraged the staff members voice to be heard from their own opinions and ideas specifically for the best interests of the residents.I enjoyed ! working for this company because it helped me acknowledge and develop my career plans major adjective by enrolling at Sinclair community college for real estate business management 2019 and was licensed real estate agent by 2021.I’m currently continuing my education at Sinclair in applied science to start my own brokerage firm.I’m thankful for the time spent with everyone there!
I had a family member there who was in good health and just needed rehab, ended up being transferred to a hospital due to lack of care.
I see many good changes with the new management staff. The social worker goes over and beyond, the administrator is accountable, the new house keeping manager ensures Covid cleaning as well as regular cleaning is done properly and the business office manager is fabulous working with the families. I encourage all staff and family members to address concerns at the time they occur and allow staff time to correct any issues.
Tried to drop some things off for my mom. Tried calling 4 times, no answer. There were 2 staff members at the desk when I went to knock at the door. They both looked at me and disappeared out of the view of the door. I tried calling again, they answered. I stated I had somethings to drop off for my mother. The woman on the phone said to come back another time because she didn't even know if anyone was manning the desk. Got back in the car and one of the cars next to me suddenly locked amd I could see one of the staff memebers peeking out the side door at me as I drove off. It's a 24hr facility and I live 45 mins away....
I have a son that is a resident of CHR. The staff has been very nice and helpful to my son and I. They are very fast at responding to his needs, for example getting him a new bed mattress as soon as possible because he is bed bound, making sure his medical procedures are accomplished on a daily basis. Overall rating for this facility is four stars.
I have lived at Centerville Health and Rehab for 3 years. I love the therapy department the most. Paige is very kind and takes me for walks outside. The staff there has become my family. I have my own private room, which is more space than I actually need. The staff is wonderful and tend to my needs.
I am a current resident, and have lived here for many years. I love this place. The staff and management are wonderful and caring people. I enjoy visiting with other residents at gatherings and activities. I would recommend Centerville Health and Rehab for your loved one.
Poor experience can’t even properly provide your medication nurses keeps miss placing or forgetting to give it to you then when they decide to give it you it is back to back with next dose and they are not polite at all and they are never staffed properly constantly complaining about the lack of staff and not focusing on the work and you have to check your own vitals because it doesn’t get done most of the time blood sugar checked and insulin delivered after you already ate and it gives improper reads after the meal staff permits a patient to go around make physical threats and bullies people then staff ignores that person is disrespectful to everyone and refuses to remove them from premises
