ER. I’m giving 1 start cause 0 is not an option. This place is TERRIBLE. Front desk is disgustingly rude and has zero knowledge on how to treat people. Don’t send your loved ones here. Go out of Chickasha to receive the best care.

Try to get a new primary care doctor with dr. Campbell but they have rescheduled Me 3 times now if I did that I would no longer be allowed to come back whatever apparently they don't want new clients took off work three times for this

Met by friendly and helpful staff. I was shown the utmost courtesy and respect. First time visit and needed to designate a primary care physician, as I am new to the area. Counseled by a very friendly and professional Physician's Assistant. This visit far exceeded my high expectations, hence my highest recommendation for utilizing this facility!

The express care team are rude and do not listen to the patient or the family. They are a very condescending group of people. My family will never use Five Oaks Express care again.

THEIR COVID TESTS ARE NOT FREE LIKE I WAS TOLD! My husband called this location and asked if the Covid 19 tests were free and was told they were. I go in to get tested, filled out a form stating I do not have insurance. I specifically asked if I needed to pay anything and was told NO, there is no charge. I just received a bill for $145 for an office visit. I called and spoke with a very nice lady in billing. She said the actual Covid test is free but there is a fee for the office visit. I'm sharing this info to help others BEFORE they get charge a completely unreasonable amount for a "FREE" Covid test. If I had known, I would have gone to CVS for an actually free test. This is no way to do business. It's sneaky and very WRONG! I'm so not happy about this.

I had an DOT physical and drug test preformed and they charged my debit card 3 times !! Two times for $140 each and one time for $40.73. Then after telling me that it only authorized for the 40.73 , I said I would pay the rest in cash , the person said she could just void the transaction and I could pay the $ 140 in cash if I wanted. Which is what I did. They never voided he two other charges of $140 each. Now they have been paid $420 for a $140 fee procedure and are trying to say it’s my fault. Wow. Would never go back to this place for any reason. I plan to pursue this as far as it takes to get my money back. Was treated very poorly and rudely by all the business office personnel.

On October 2nd, 1984, I remember being here, although I am not sure it had the same name then as it does now. The then Chickasha High School Nurse, Linda Pierce was present ONLY at the high school to let my mom and me know I could not attend Chickasha High School beginning in the 11th grade, until I was given either a Measles or Mumps vaccine, or shot for safeguard. She told us this after she herself sifted through a stack of my educational attendance records, and she mentioned the one specific shot that I needed, even though I did not have any active case of Measles, or Mumps. I had already had these when I was very young, and appears as archived information in my documentorial baby book. So, the above information was told to us on October 2nd, 1984, when I was 22 days short of turning age 17, and still able to go on developing and growing. However, all this was about to change that very day. My mom and I went to this location, where Five Oaks Medical Group now is, and a separate administering Nurse on duty there tells both me and my mom then known as Wanda Adams: "We're all out of the one shot he needs, however, we do have a combination dose shot, M.M.R., Measles, Mumps, Rubella vaccination shot". I said "o.k.", and the Nurse there at the clinic began to give me the shot in my right arm. This was done and over with, and my mom Wanda (Davis) Skelly Adams then, and I started walking toward the door to exit the building, when I began to have AN ADVERSE REACTION with Tinnitus [ringing in the ears], and I mentioned that, then and there and my mom Wanda said, "Oh, come off it", and the Nurse said "Oh, Robert, you are just hyperventilating, you are just scared of the needle", my mother thought I was a put-on/faking it, and she was wrong about that! I continued on towards the exit door with my mother and the ringing in the ears intensified, and I also went blind, not able to get out the door. I was so disoriented, I collapsed in the floor just to the left of the exit door by just a few steps. I will never have another M.M.R. combination shot again! I could do nothing but stagger out to the vehicle, and I could not even sit up in the cars front passenger seat, and my mother Wanda Lee (Davis) Skelly Adams, expected me to attend school that very morning. I could not, and I did not. My time came to attend the 11th grade there at Chickasha Junior-Senior High School as it was then known as, and my most of my learning began to fail, and almost all of my grades were D's and F's, nothing like my normal. This went on from early October 1984 through sometime into February 1985, and I could not even stay awake to learn one of my most important courses, Drivers Education, which I had to drop out of because I simply could not stay awake and effectively learn. I have since learned: M.M.R., Measles, Mumps, Rubella combination shot vaccine, is unsafe. It has a known history of being the initiating culprit that will cause Autism to develop in younger children, or students. There has also been deaths reported. So, I am now age 53, and I am no longer 6 feet 3 inches in heighth, and I am Learning and obviously Developmentally Disabled, although my adaptation is limited daily. I am now a barely surviving Dose Filter, still entirely on my own, from birth, and I do not reside in Chickasha, Oklahoma. I moved from there to Bakersfield, California, U.S.A. in probably early March or early April year 1985. This is my brothers birthland, he was U.S. Army Specialist 4 STEVEN GENE SKELLY, a fatality in the Vietnam War Conflict on November 13th, 1969. 3 of 7 of my immediate family members are now gone/deceased, in chronological, specific dated order, they are: STEVEN GENE SKELLY (11/17/1948-11/13/1969). JOHN PRESTLEY/PRESLEY SKELLY (05/07/1925-10/16/1998). WANDA LEE (DAVIS) SKELLY ADAMS ROCCO (07/07/1927-01/14/2020).

Dr. Thomas Jarvis is excellent, personable and most of all he listens. If your a veteran you want Doc Jarvis.
