My mother and I were going to tour Jefferson Gardens until I saw on JGs FB page shekita gray was a nurse there. Knowing from past experience with her what kind of dishonest and unskilled nurse she is I would never leave a loved one in her care and urge anyone looking for a place for their loved ones to avoid this place if possible. Iv personally witnessed her forge documents, abuse staff, and put residents in immediate Jeopardy. I hope this saves some residents from her treatment.
I called earlier in the evening to ask a couple of questions and guess I was too late, the employee barked at me "they don't tell us that stuff" and that they had "2 halls all by themselves to get back too" a simple call back during office hours would of sufficed. It's terrible they put so much on one person like that but I don't think I want my papa in a place like that.
After hearing positive comments from my dad‘s hospice nurses about Legend at Jefferson Garden‘s I decided to move him here. The staff at this facility have been amazing! Every staff member does their job efficiently and effectively. When my dad moved in the Director asked about his favorite snacks and activities. She made a welcome basket complete with an OU football schedule. The aides who dispense his medication are on top of his needs and check on him regularly throughout the day. The nurse is fabulous and easy to work with. My dad‘s favorite meal of the day is breakfast. He has truly enjoyed having biscuits and gravy, eggs, sausage, pancakes and all his favorites for breakfast. Even the staff who does the laundry and cleans the rooms go over and above. They are very kind to hang up my dads clothes since he has a difficult time walking. I have yet to see dust in his apartment. The social Director plans engaging and fun activities as well. This is the fourth retirement center my dad has been out in the past five years. For him, Legend is certainly the best!
Thank you Legends for taking care of our mother and mother in law for the last 5 years. Legends @ Jefferson Gardens is a wonderful facility with passionate care! 5 stars all the way!
Overall a very positive experience. Most of the staff were fantastic, kind, competent, and attentive. There were a few staff over the course of 3 years that weren't up to the same level, but they usually didn't last long. We had some issues with the food at times, but they improved. The medical director Tomee was especially great to work with. Special call outs to Doris, Topie, Mellisa, and Stacey also. The program director LJ was wonderful and so caring. I wish I could give a 4.5 rating.
My mother had the pleasure of spending time at Legends in 2021. The entire staff, from Andrea to Tommy to Stacy and their entire team, made the stay as comforting as possible for mainly my mother but also all our family members. They do truly care and love their residents. The facility was always clean in all areas. Numerous daily activists were available for all the residents that wanted to participate. Kitchen staff was friendly and professional, providing three freshly cooked meals a day. Without question one of the finer facilities in the region. Highly recommended for anyone in need of the services they provide.
We transferred my Mom from skilled nursing to Legends due to her having a stroke in October of 2020.My mom had regained some of her mobility back through PT at the skilled nursing facility before being taken to Legends. We had done extensive reviews of our options before selecting Legends. We heard nothing but good things about the facility and the video tour was impressive. Unfortunately, from the time we brought her there until the day she died 79 days later her condition completely reversed. She had had multiple Covid tests while she was in the skilled nursing facility and had one that was negative right before she was transferred to Legends. The skilled nursing facility never had a Covid case while she was there. Legends insisted that she still go into a 10 day quarantine. During this time of isolation she did not receive any PT to further the progress she had made at the skilled nursing facility and became totally immobile once again. At the skilled nursing facility they had allowed window visits so we could at least see her & talk on cell phones to each other. Legends flat out refused to even consider that option. The thing that became quite clear over the 78 days she was there was that Covid had turned this beautiful facility into an expensive prison. There was never any privacy allowed with your loved one and you were allowed to visit for a short 20 minute session once a week for a maximum of 2 people. This was at the front entrance where it was noisy and people were coming and going. The corporate cultural was very obvious when we learned that the director was having to also cover for another facility that had lost their director. She was a nice enough lady, but the corporate rules always superseded any personal allowances for a resident’s needs. The other issue was a shortage of staff. This excuse was used for having just one visit a week for twenty minutes and we can only do it Monday through Friday because we have reduced staff on the weekends. So family members who have to work during the week in order to pay the $4600 a month for their loved one to stay there are just out of luck.My mom died from malnutrition from not eating. For whatever reason she just wouldn’t eat and obviously you can’t survive without eating. The thing that drove myself and the other family members crazy was the lack of accountability of the facility. Again, the closest thing I can compare this to is a prison, because of the total release of accountability due to Covid. After I visited with the doctor, whom the facility used, I contacted the director to get final visits approved before mom passed way. She informed me that she wasn’t close to dying yet, and I told her that the doctor said that she was and that we wanted to be able to go to her room with no time limits. I also told her that their visitation at the front entrance was completely ridiculous. To her credit she relented and agreed we could start unlimited visitation in her room starting the next day which was Thursday, she died that Saturday morning at 10:30. I hope no one has to go through what our family did with this corporate facility. Even though we thought we had done our due diligence and was moving her into a premier assisted living facility it turned into a Covid corporate nightmare.
A great change of venue in selecting a convenient store
I joined the team at Legend at Jefferson’s Garden in May 2019. I couldn’t be happier!! The orientation and on-going training has been the best I’ve ever received! Thank you to Legend Senior Living for creating the structure and guidance needed to help me be successful! I feel completely supported by all levels of management including the owner, Tim Buchanan.
Our experience with Legend at Jefferson’s Gardens has been wonderful! From day one when we first toured the community we knew this was the right place! It has been terrific to get to know all the associates, other residents and family members. Everyone is so helpful, friendly and truly caring. It’s a great place to call home!

Selection Criteria
All the information you see here is based on our analysis of internet reviews across the seven major review platforms most used by consumers during their healthcare provider selection. The platforms we selected are Google, WebMD, Yelp, HealthGrades, Facebook, RateMDs, and Vitals. The analysis algorithm uses the compiled data of average score, recency of reviews, number of reviews, and the sentiment expressed in the written comments. It then assigns weight to each parameter based on the available research on how consumers use reviews in their doctor selection process. This report is designed to help identify the doctors with the best overall review score in an area and does not reflect the doctors' competence.
