Dr. Chlebowski is top notch. I received excellent care in a clean facility. The nurse and receptionist were very caring. The dr. Removed my very ingrown nail without any pain. I highly recommend this dr. I am so thankful to have found him.

He was not particularly warm or comforting. I left FP because I had gone for bloodwork in their office and the woman hurt me; I told her to stop and immediately left and went to the ER. They said the phlebotomist gave me a hematoma. I had a bruise for a good month. NOT FUN. Chlewbowski saw me in pain and leaving but didn't even bother to check on me so I figured he didn't really care. 4 years later I am trying FP again but with a new doc there. Hopefully that gal is gone and my new doc is nicer!

FAMILY PRACTICE CENTER lets Dr. Chlebowski drop patients like hot potatoes. After being a patient for 5 years, he will not discuss it with you or refill Rx so you have time to find a new GP. Have to go to the emergency room to get refills - It has happened to me and others I have talked to. NOTE: Rx is NOT a pain medicine, but one that cannot be stopped suddenly without possible danger to patient or side effects.

My first visit to the skin care center for Botox. Dr. Chlebowski gave painless injections and my visit was relaxed and not rushed. Looking forward to seeing the results develop in the next few days but I can already say I'll be back.

I would never go to anyne else. He has proven time and again to be caring, accurate in his diagnoses, willing to listen, and willing to give each appointment as much time as needed. The fact that he made home visits during my husband's terminal illnes puts him over the top.

I felt an immediate connection to this doctor; he listens to you, and he explains the options and reasons for them; not like other doctors I have dealt with who simply write you a script and dont really tell you much. I trust him; he found a diagnosis that was looked over many times, and I feel I am on the road to a healthier lifestyle. So far so good! Thank you Dr!
