Brookdale At Home Nashville's Online Review Report Card

A huge thank-you to the folks at Brookdale in Kaizer, Oregon. They took such great care of my Mom, and she loved it there. We were so happy she was able to stay there through hospice and her passing. I definitely recommend this Brookdale, and we had a similar lovely experience when she was at the Brookdale in Redmond, Oregon.

NEVER give them your phone number. They hound you forever with automated calls, ignore requests to be removed from their lists and don't answer their phones.

Last evening my dinner delivered consisted of 4 pieces of Pork Fat. No meat was attached. I also received potatoes, no vegatables. This is assistant living at it's finest. Marcia Schwartz Brookdale Chicago

I tried to volunteer at the Flagstaff Arizona Brookdale facility in my community. After 40 years working in healthcare I understand the importance of connection. I also worked 16 of those years in long term care and understand the challenges of making the residents feel content and valued. I went through all the motions at this facility (background check and TB test) and left a voicemail for the activities coordinator. She never returned my call. Never. Each time I went in, there was NOTHING happening in her department. It's a shame your company chooses to isolate our seniors and keep the community out of your facility. Wonder why that is...perhaps I will try to discuss my concerns with the local ombudsman. I am very concerned.

Morehead City community has a long way to go before family can trust them to take care of their loved one.

My husband is in the Brookdale Skilled Nursing facility in Greenwood Village Colorado. Nurses, PA, PT, OT and PCT wonderful people BUT please train your Social Worker to STOP calling your patients family members LIARS! Teach her to NOT suggest the PT to AMA because she CAN'T do her job! I am the wife I had to find O2 in our area in AZ and then was told I lied. The medical supplier then had to call the SW to confirm I did talk to them and the supplier was waiting for a script! But she said well its to late to fax a script. We are NOT CO residents but AZ residents my husband is terminal and wants to come home and has a very important appointment with his primary care specialist but we now have to reschedule a very important appointment in our area because she can't figure out how to get O2 tanks for traveling! I just know she'll have my son who WORKS go get the tanks! Instead of having the tanks delivered to Brookdale Skilled Nursing in Greenwood Village CO. ready for travel. My husband is now heart broken he's there in what he calls JAIL! another week! To many excuses and team meetings when everyone is ready to discharge,BUT HER! Instead suggesting AMA! I made calls to find O2 and my son will be pickup guy for enough tanks to travel home. But to basically be called a liar and never get an apology! By the time my husband gets discharged... we don't know if he'll be alive. My husband said he's going to die before he comes HOME! And another team meeting to discuss what? How he's going to die? NEVER EVER call your pt or the family a LIAR! Especially before facts can be confirmed! Get people home when you KNOW they have Dr appointments that are literally life and death. And yes we know you don't know the total reasons why he is there and not in AZ but to treat family like gum on your shoe hmm?

Our experience has been one of the worst looking for a place helping my brother dealing with Alzheimer’s. After two appointments etc. on the admission day they denied services very UNPROFESSIONAL!!
