Physician put into husbands medical records things that were never discussed, left out meds that the dr put him on, conveniently are missing labs, and on more than one occasion. Only taken us two visits to realize it's time to change providers. And on a separate note, I had tried to get in to establish care, only to be told that our New Patient coordinator will call you, and that was over 6 weeks ago.

I have gone to Dr. Chavez for several years now and He makes me feel at ease and knowable of my health care plan. He takes the time to explain everything and listen to my concerns. He really is the best doctor around. And all of his staff is friendly and welcoming from the moment I walk through the door.

Great experience today, from the office staff to the nurse and especially Dr. Chavez. I found my primary care Dr.! If you need a Dr. who actually cares I highly recommend Dr. Chavez!

I was invited to visit the doctor to get acquainted and I was impressed with Dr. Chavez. I've since had a first appointment where I visited the in office lab and provided lab samples and had a number of tests done. Looking forward to receiving test results.
