You are in good hands and are lucky to have Dr Crookston as your Dr as he truly cares about your well being and health!

This place is amazing. The front staff have always been helpful, caring, and kind. They help with anything you need and do it as quick as they can. I've seen Dr. Crookston for about 6 years. He is a wonderful doctor. He listens patiently, talks through your issues, he makes sure you are feeling well on any medications and helps find the right solution for you. He's always been so kind to me, he's kinda funny if you get him going, lol. He's taken care of me and I always look forward to speaking with him. I seriously don't know what I would do without him, so much so that I asked him to give me 5 years notice before thinking of retiring.

If I could leave zero stars I would! If you are considering using Dr. Crookston as a psychiatrist, please find someone else! This doctor killed my ex-husband. He kept my ex on suboxone and told him he could keep taking it for as long as he wanted and after 8 years he is dead. He stopped caring, he stopped living his life and he stopped having relationships with pretty much everyone. My family has been destroyed because this doctor refused to see what the drug was doing to his patient. This doctor did nothing to help him, all he did was encourage the use of suboxone and take a co-pay. If you are looking for serious help please consider finding someone else!

I have been seeing Dr Crookston for over 10 years now. He has helped me with some rough things in my life. Right now I doing very good thanks to his treatment. I hope he doesn’t retire to soon!!

I've been seeing Dr Crookston since i was 19 & they were out on Fort Union. So that's 7 years. He has seen me through first realizing I had an addiction problem, through going to Rehab, through sobriety & relapses & through my other mental health road I've walked all these years. I honestly could not ever ask for a better Psychiatrist. He listens to what I have to say, he encourages me to decide how I want to proceed with medications & treatment. I live in all the way in Herriman, but this man is truly so incredible that driving an hour to see him in rush hour traffic isn't bothersome at all. I've had Doctor's in my area, specifically PCP's who try telling me I can switch my meds to them so I don't have to drive so far, but I always say no thank you. Reason being; Dr Crookston knows ME. He knows all of my medications, he knows about my lifestyle, he knows my personality & how I deal with stress & most of all, he understands the medications he is prescribing & how they interfere with other things. Not all doctor's can say the same. He's even been able to educationally discuss "homeopathic remedies" with me so I could understand whats safe, whats worth it, their makeup & even the hype on certain remedies. I could not be anymore satisfied with my care here. He works with you on how often you see him based on YOUR progress as well. When I was 1 year clean, we were seeing each other monthly. When I hit 2 years clean, we switched to every 2 months. Now I'm over 3 years clean so I only see him every 3 months, but still have the ability to see him sooner if need be. Its also really nice to use the patient portal when you have basic questions or requests for your Psychiatrist. I highly recommend The Avenues psychiatry & have never had any complaints about any of my care because its always been outstanding.

Doctor Crookston is simply one of the most perfect specimens of a human being that there is he is an awesome doctor whose insightful and excellent treatment modality equate to a wonderful experience I feel better every time I'm leaving his office than I did when I arrived and that means a world of difference for me Sincerely Brook Walton

I have seen this Dr twice and is very good at what he does. He is always knowledgable about new medication, changes to the RX med forms, for being an older experienced Dr. I was surprised at how often he keeps updated to the ever changing treatment options. He even has toys in his office for kids and is very good with my kids. He stocked up on army guys for the little boys of his patients to keep them happy and playing the entire visit.

Dr. Crookston has been the single most helpful person in my recovery. I have been clean and sober for almost 7 years. These past 7 years have not all been easy, I have had to learn to live all over again. Each and every obstacle I have encountered I encountered with Dr. Crookston's guidance. He's always willing to take the time to help walk me through each problem I have come across. Always offering advice and complete information so that I can make an informed decision. If a person is truly ready for sobriety, Dr. Crookston will be the helping hand that make sobriety attainable.
